Well, we'll get more into that in a future episode. But okay, is there any homeruns stories with a patient who's had a fat transfer with you that is like, so fantastic.
Dr Payman Danielpour 31:46
Absolutely. Early on, in my practice, probably a year or two into my practice. This lady came in from, I believe, the Philippines. And she had a weird kind of like flesh eating bacteria. It wasn't necrotizing fasciitis. I don't know what it was, but the right side of her face had zero fat, it was like eaten away. And you can tell on the outside too, that there was some scarring. So she came in to see us and she's like, What can I do to fix this I go, the only way to fix this is fat transfer. And we took her in one session, fat transfer to her face. And I think I saw her a couple of years ago, she still is retained it it is still and the skin on the outside is now looking amazing. And again, this has nothing to do, it was cosmetic, but it was more of a reconstructive case which, which is why it's so great, because it's not like she did anything to have this happen. But we had a solution for it.
Dr. Vicki Rapaport 32:45
I also love that you use that as your homerun case because it's a medical problem. Maybe she had morphia maybe she had CUDA sobre. So there's so many people with these, you know, serious medical atrophy diseases where fat transfer could work beautifully. Obviously, you have to make sure it's all burnt out. Before you do that. I imagine she had had that for years and years. But I love it use that.
Dr Payman Danielpour 33:07
We sent her to infectious disease to make sure that that was completely cleared, she was ready to go and then we inject her. And she was a really pretty, pretty woman. So you've really kind of, you're like, Wow, I can't believe this. It's such a great case. Because you look at it, you're like this works. I had a similar patient who had who had a congenital anomaly of her buttocks where one side of her buttocks was formed, and it was a really nice round shape. And the other side was indented and flat. And she was like she was like 19 years old. 20 years old, came in with her parents. And we did a fat transfer to her and it looks it looked amazing just took fat from from her flanks I think processed and put it into the area that she had the concavity. That's the beauty of using something like fat transfer because you know that it's going to last and also mentioned how many syringes of filler I had to put in someone's buttocks or even in this lady's face. You put in five syringes, it may look good. And then a year later, she has to come back and get it again so it lasts longer. It has regenerative qualities I love using fat for just just about everything
Dr. Vicki Rapaport 34:16
I think you should give a fat transfer class for dermatologists who want to learn how to do fat transfer
Dr Payman Danielpour 34:21
love to.
Trina Renea 34:21
Can dermatologist do fat transfer?
Dr Payman Danielpour 34:23
Of course they can
Dr. Vicki Rapaport 34:23
Yah, we do liposuction, fat transfer.
Dr Payman Danielpour 34:25
I mean it's really like the easiest thing in the entire world honestly, like if it will be like it's easy to teach and have you do it can you do it like this? Especially if you have the aesthetic eye and how to inject a face then just all you need to really learn is the harvest right which is easy. You just come down to the operating room do it with us,
Dr. Vicki Rapaport 34:45
okay
Trina Renea 34:48
Can I come watch?
Dr. Vicki Rapaport 34:49
I also feel like the women who donate their ovaries and you know make $5,000 in college because they need the money. I feel like we should be able to donate our fat even though I realized you can't do That's the obviously there's mass
Dr Payman Danielpour 35:02
Do you know how many people come in and they're like, can I get my fat to her for breast and I'm like, doesn't work that way
Dr. Vicki Rapaport 35:07
Ah, wouldn't it be great. clean it up. And
Dr Payman Danielpour 35:12
so you know, you've heard of face to face transplants and things like that, right?
Trina Renea 35:17
Face transplants? Do you mean like taking someone's face off and putting someone elses face on it?
Dr Payman Danielpour 35:21
Yah, from like a really bad burn. So this is the latest and greatest topic in plastic surgery is doing tissue transfer. We've been doing it forever, free tissue transfer, but doing it as a transplant. So for years and years and years, we've been doing it from ourselves. So very well known breast cancer reconstruction, you take the belly as a tummy tuck, you pull it off, and then you reattach it to the vessels of the heart. And they have breasts from the belly, okay. It's called the DIEP flap or free TRAM flap or a TRAM flap. Now, and this is really fascinating because the new chairman of Plastic Surgery at Cedars Sinai guy by the name of Curtis Cetrulo, who came over from Harvard is like one of the guys on the forefront. Now you can take, for example, people that are young men that are in war that come in and they have their genitalia blown away. Okay, they come back, think about it, everything else is working, but they have no genitalia. You can take that from a deceased person who's donated their body and do a tissue transfer right back.
Dr. Vicki Rapaport 36:31
An organ transfer
Speaker 1 36:33
Absolutely. Like a kidney. Yeah, like a kidney. And he says that within two years, it has sensation, erection and ejaculation properties. Insanity. Because imagine a young guy that goes to war comes back and he can't have, I mean, it's, it's it's a game changer, right? So it's, it's truly fascinating. So so not only are we...
Trina Renea 36:57
But the dead persons tissue has to still be alive. And
Dr Payman Danielpour 37:00
So they harvested it. Absolutely. And they bring it and they make sure it's viable. And they do. For example, he did an arm from here. That was that was stuck in a spaghetti maker, and they couldn't do anything with it cuz it was torn apart.
Rebecca Gadberry 37:14
And you're showing mid arm, for the people at home, who cant see
Dr Payman Danielpour 37:15
It was mid arm, and literally took someone else's arm that was in a bad car accident that donated their body, put it right, showed videos, the guy was moving his hands. This is the kind of stuff when we talk about medicine that like it's just the game changer. I mean, it was blowing my mind.
Trina Renea 37:31
That's so exciting.
Dr. Vicki Rapaport 37:32
on that note I am so excited with all that information with Dr. Danielpour today. Did we have a fabulous time with him?
Trina Renea 37:41
that was amazing. It's really good.
Dr Payman Danielpour 37:43
I came on Facially Conscious to talk about genitalia, but yes, sorry
Rebecca Gadberry 37:48
I'm not gonna say a thing because I'm usually the one that does a stupid joke here. But there's to many.
Trina Renea 37:56
No, we had a guest on, Dr. Kotler who talked about the phenol peel as a way instead of you know, to help with the facelift, which is like a dying thing now too
Dr Payman Danielpour 38:10
Phenol Peel is, is fantastic. It really is. It's the best peel. However, it's very old fashioned. And it carries a little bit of a high complication rate. There is nothing, hands down that you can get a better peel from. By far. I mean, it's not even a question about it, but the problem is
Trina Renea 38:11
people aren't doing it anymore
Dr Payman Danielpour 38:34
you've got to know what you're doing. For someone like Kotler, who's been around for a long time, he probably does it to perfection. Now you have lasers to, I mean you have fractionated co2 laser. Really a safe laser, and it gives you pretty good results. It doesn't go as deep as a phenol peel. But it does give you pretty nice results.
Trina Renea 38:54
Right? And I love learning about fat transfer as an option to fillers too, in the face.
Dr. Vicki Rapaport 39:01
a resident plastic surgeon in the making.
